Learning disabilities mortality review programme

Welcome to the consultation exercise for the Learning Disabilities Mortality Review (LeDeR) Programme.

The aim of the LeDeR Programme is to make improvements in the quality of health and social care service delivery for people with learning disabilities to help reduce premature mortality and health inequalities in this population, through national and local reviews of deaths. The Programme is commissioned by the Healthcare Quality Improvement Partnership (HQIP) on behalf of NHS England and was one of the recommendations of the Confidential Inquiry into premature deaths of people with learning disabilities (CIPOLD).

The aim of this consultation is to clarify:

  • The core data to be collected at the notification of a death, at an initial ‘light touch’ local review, and in a full multidisciplinary review of the death of a person with learning disabilities (LD). Full ethical approval will have been obtained for agencies to share this information.
  • The criteria for which deaths are would require an initial ‘light touch’ local review only, and which will require full multiagency review.
  • The definition of key terms used in the Programme
  • The standards against which ‘best practice’ will be measured.
  • There are 13 pages to this consultation document. The navigation tab on the right hand side of the screen will indicate the content of each page and take you directly there.

LeDeR appreciate your help with this and would like you to contribute as much or as little as you can. Please don’t feel that you have to respond to every section or question. You need to fill in the first page for us to let us know about yourself, but then you can choose which sections to complete. You might, for example, only be interested or able to comment on the definition of terms we will be using (Page 10) so if that is the case you can just go straight to the relevant page and only complete that section after you have filled in the first page. Alternatively, you might want to go through the whole of the consultation document and respond more fully.

The whole document will take approximately 30-40mins to complete. You are able to complete the document in stages, returning to it at a later stage if that is more convenient, as long as it is submitted by the closing date of 9am on 10th August.

It will be easier for us if you could complete the consultation document online please. We have tried to make it straightforward for you to do so.
